The Hallowell ADHD Centers do not accept insurance, Medicare or Medicaid.

We encourage you to contact your insurance company directly to determine details of coverage and reimbursement. You should be aware that insurance companies often require a clinical diagnosis, treatment plans, treatment summaries, or copies of a patient’s entire record. We shall provide this information if you sign a release form specifying who is to receive the information, type of information to be released, the purpose for which the information is being requested, and for what period of time the authorization is valid.

Upon payment we will give you an invoice which includes all of the diagnostic codes and procedure codes as well as clinician information needed to submit to your insurance company. Many patients submit these to their insurance companies and receive some reimbursement.

This agreement is being provided according to the Health Insurance Portability and Accountability Act. We will be happy to provide you with a copy of this agreement if you request one.

Cancellation policy

To best serve the many patients who come through our doors, we adhere to strict cancellation policies. Reminder messages will be sent 24 hours before a scheduled appointment. Please note, however, that confirmation messages are a courtesy and that you are still responsible for keeping track of your own appointment, Not receiving a confirmation message does not excuse you from the late cancellation policies

To cancel or reschedule and avoid being charged for your appointment, you must cancel by 10:00 am the day before the appointment. For Monday appointments you must cancel by 10:00 am on Friday. Appointments cancelled after the deadline will be billed at the full rate of the appointment.

A scheduled neuropsychological testing must be cancelled at least one week before the testing is scheduled. One half of the testing fee is charged one week before the testing. If you give less than a one week notice to cancel, you will lose the deposit.

Late cancellations due to unexpected illness or other emergencies will be evaluated on an individual basis by the clinician.

Payment

The Hallowell ADHD Centers accept cash, checks and credit cards (Visa, MasterCard, American Express, Discover). Payment is due at time of service.
